| Follow these simple instructions to get started:

Sign your name on the signature panel on reverse side of the card |

| Step 2 |
Complete and fax/mail the registration form to 03- 2162 3067 or 2164 3344, along with a photocopy of your NRIC (both side)/passport. |

| Step 3 |
Allow 4 working days for the Card to be activated before using. A SMS notification will be sent upon successful registration. |

| Step 4 |
Top-up value into your card before making purchases at AmBank/AmAssurance branches nationwide, AmBank Cash Deposit Machines, participating e-pay outlets where you see the NexG Recharge logo, via cheque payment and fund transfer from your AmBank account, InterBank Giro , or MEPS Bank ATM card at AmBank ATM. |

Accept the Terms and Conditions |

Select account type (please select ATM/Credit Card) |

Enter NexG PrePaid MasterCard number followed by Web PIN |

Create Log in ID and Password |

Complete the Registration Process |
